The Rhode Island Stroke Coordinator’s Network:
Increasing Collaboration, Communication and Community among Stroke Coordinators


Description

The Rhode Island Stroke Coordinators Network was established in December 2008.  Participation was open to acute care hospitals and public health partners.  The preliminary mission of the group was to build a statewide community for stroke coordinators to improve patient care statewide.  The group identified collaboration, communication, and education as the preliminary areas of focus.

Accomplishments

To date, the Rhode Island Stroke Coordinators has been able to accomplish the following:

§  Obtained commitment from all 11 hospitals statewide and 80% participation rate

§  Adopted and implemented several statewide projects

§  Hosted a Continuing Education Event that was attended by over 100 nurses statewide

§  Supported the passage of the Stroke Prevention and Treatment Act of 2009 which established a statewide stroke system of care

§  Increased collaboration among hospitals and public health organizations

§  Increased participation in the Northeast Cerebrovascular Consortium

§  Obtained grant funding from the RI Department of Health

§  Supported other statewide stroke efforts including public education programs, professional education programs, Stroke Task Force, Heart Disease and Stroke Prevention Program, and the RI Department of Health

§  Created workgroups in the following areas: Community Education, Nursing/EMS Education, Paitent Education and Discharge, and Rehab/Support Services


 
Evaluation

In July 2009, the Network conducted a survey to measure the satisfaction of its membership after 6 months involvement and determine the future direction of the group.  The survey was administered over the period of four weeks.  A representative from each of the 11 hospitals and the 4 public health organization received the survey by e-mail.  The survey was composed of ten questions that were either multiple choices or opened ended.  The survey received 8 out of a possible 15 responses.  76% of those who responded to the survey classified their experience as excellent or good whereas only 24% described their experience as fair.  The survey asked about the primary responsibility of the Network offering the following variables:  sharing best practices, fostering collaboration, networking, addressing the state stroke plan, hosting educational forums, and other. Sharing best practices received the highest response of 24% followed by networking which received 21%.  When asked about the future direction of the Network, respondents were offered the same variables as the first question.  25% believed sharing best practices should be the focus of the group moving forward followed by fostering collaboration which received 21% of the response.  This survey analysis identifies specific areas of focus for the Stroke Coordinators Network and the currently level of satisfaction for the respondents.  More should be done to better understand the perceptions of the stroke coordinators who participate in the network as well as to better identify the needs of this group.  Moving forward, the Stroke Coordinators Network should develop a plan increase opportunities to share best practices and collaborate.

 
Rhode Island Department of Health

In 2007, the Rhode Island Department of Health received funds from CDC to support a state heart disease and stroke prevention program. It also received CDC optional funding to implement a demonstration public health project that will improve the management of high blood pressure and high blood cholesterol and improve quality of care in the healthcare setting by enhancing the existing Rhode Island Chronic Care Collaboration in nine community based health centers in the state.
